Employed Members
Working in government or private sector
(e.g. Office workers, household helpers, drivers)
Self-Employed / Voluntary
With own source of income
(e.g. Freelancers, small business owners)
OFWs
Working abroad (land-based or sea-based)
(e.g. Seafarers, overseas workers)
Lifetime Members
Retirees with 120+ monthly contributions
(e.g. Government or private retirees)
Sponsored Members
Premiums paid by government or private sponsor
(e.g. Indigent families, LGU-sponsored)
Senior Citizens
Automatically covered by law (RA 10645)
(e.g. Filipinos aged 60 and above)
π How to Register
Option 1: Online
Go to π www.philhealth.gov.ph
Click Online Services β Register
Fill out the PhilHealth Member Registration Form (PMRF)
Upload a valid ID and required documents
Wait for your PIN and Member Data Record (MDR) via email

Β Option 2: Walk-In
Visit the nearest PhilHealth Office (LHIO)
Fill out the PMRF form
Present a valid ID and supporting documents
Receive your PIN and MDR
