Information for New Patients

Please call the number on the back of your insurance card and change your primary care physician to Philip Fu, MD. If this is not done prior to your visit, your visit will be rescheduled. If you do not come to your first appointment, you will not be permitted to schedule another. You may reschedule your first appointment only once with valid cause. 

Please arrive at your appointment 30 minutes prior to fill out the paperwork and to complete your office registration. Please bring photo identification with you along with your insurance card and co-pay. Please bring a list of your medicines and medical history. 

Please come to the physician with an open mind. The best physician-patient relationships are built on mutual respect. Please understand that traditionally physicians ordered medicines, lab tests, and imaging based on their clinical judgement and not based on patient request. Please know that when this occurs, the patient-physician relationship is altered, as the patient steps into the role of physician to self-treat. We do not do chronic pain management and controlled substances are given only in limited supply.
