NOTE: This practice operates on a 'fee-for-service' basis. Therefore, no insurance assignment is accepted including no Medicare or Medicaid. Payment is expected at the time of service. For 'In Person' visits payment can be made with cash, check, or other electronic payment service. For Telehealth visits payments can be made with an electronic payment service with payment in advance required to confirm the appointment.
