PCM/MAS-AMiCUS NCMC ("we" or "us") is committed to protecting the privacy of individuals ("you" or "your") whose data is collected through the PCM/MAS-AMiCUS NCMC Profiling System 2024 ("System"). This Data Privacy Consent Form ("Form") explains how we collect, use, disclose, and store your personal data. By submitting your information through the System, you consent to the data practices described in this form by whatever means in accordance with Republic Act (R.A.) 10173, otherwise known as the “Data Privacy Act of 2012” of the Republic of the Philippines, including its Implementing Rules and Regulations (IRR) as well as all other guidelines and issuances by the National Privacy Commission (NPC). 

We will not disclose your personal data to any other third party without your consent, except as required by law or to protect the rights, property, or safety of PCM/MAS-AMiCUS NCMC or others. We will retain your personal data for as long as necessary to fulfill the purposes described in this Form, unless a longer retention period is required or permitted by law.

We may update this Form from time to time. We will notify you of any changes by posting the new Form on the System.
