Qualification and Requirements

Endorsing parties shall submit the following documentary requirements:

1. Letter addressed to the Chairperson, endorsing the Endorsed Individual and indicating the name, credentials, and qualifications of the Endorsed Individual; 

2. Notarized Accomplished Endorsing Entity Profile Sheet; 

3. Securities and Exchange Commission (SEC) Registration, certified as true and correct by the BSO or CWA’s Corporate Secretary or by the SEC;

4. List of Officers and Member Individuals or Organizations, as applicable, certified true and correct by the BSO or CWA’s Corporate Secretary; and 

5. List of Programs and Projects, certified true and correct by the BSO or CWA’s Corporate Secretary.
