Patient Education and Safe Discharge Toolkit
The toolkit is aimed at helping nurses address patient education gaps that result in medication errors, poor medication adherence, unnecessary readmissions, and patient safety risks. It is meant to convert evidence from the three assessments into practical training modules and checklists that standardize the teaching taught on discharge to ensure patient comprehension and prolong follow-up with home visits using telephone reinforcement and online support. The safety improvement initiative aims to enhance discharge communication with teach-back plain language materials, health literacy screening, medication reconciliation, and targeted postdischarge contact to decrease adverse events and readmissions and enhance patient autonomy in controlling their care. The toolkit provides validated tools, sample curricula, step-by-step workflow, and helps nurses apply measurable interventions and demonstrate impact to leadership. Target audience: Nurses, nurse educators, and quality and safety officers leading or supporting transitions of care and education programs.
