Poor patient education has continually posed a critical challenge to patient safety, care outcomes, and, broadly, the overall efficiency of the healthcare system. Poor comprehension of medical instructions, treatment plans, and subsequent follow-up often results in medication errors and non-adherence, leading to readmission, especially among vulnerable populations with low health literacy or a lack of recourse to resources. To tackle these issues, this toolkit collects 12 annotated resources on evidence-based strategies, innovative technologies, and interdisciplinary approaches to improve patient education. The selected resources cover various approaches concerning patient education, including communication techniques, digital tools, cultural competence, and social determinants of health issues. This collection is a comprehensive guide for nurses and other healthcare professionals to implement and sustain effective patient education practices, bridging critical knowledge gaps and fostering equitable healthcare delivery.
