INTRODUCTION

The Patient Information Department, previously known as the Medical Records Department is a Department under the clinical support services established in 2012 at the UiTM Medical Specialist Centre (PPUiTM) Sungai Buloh and Selayang When the Al-Sultan Abdullah UiTM Hospital (HASA) in Puncak Alam commenced operations in 2021, the department's name was officially changed to Patient Information Department. This department plays a crucial role in managing comprehensive information and data to demonstrate the progress of medical levels and hospital services.


The department is responsible for activities related to the management of patients' medical records, both physically and electronically. This includes the issuance of patient information, medical report management, hospital statistics management, classification of diseases/clinical procedures based on the ICD standards, and all aspects related to the role of Patient Information Department as the hospital's information manager.

 

There are four units under the Patient Information Department, namely:

              I.   Management of Patient Records and Electronic Medical Records

This unit is responsible for the management of patient files and electronic medical records. They ensure the proper organization, storage, and retrieval of physical records and oversee the implementation and maintenance of electronic medical record systems. 


            II.   Management of medical Reports

This unit is responsible for managing medical reports generated by healthcare professionals. They ensure the accuracy, completeness, and timely delivery of medical reports to the relevant stakeholders, including patients, healthcare providers, and external agencies.


           III.   Casemix and Health Informatics

This unit focuses on casemix management, which involves the classification and grouping of patients based on their diagnoses and procedures. They also handle health informatics, which involves the collection, analysis, and interpretation of healthcare data to support decision-making, research, and quality improvement initiatives.


          IV.   General Administration

This unit provides administrative support to the Patient Information Department to ensure the smooth operation of the department.


Starting from January 2022, the services of Patient Information Department at PPUiTM Selayang were closed in line with the transfer of clinical services from PPUiTM Selayang to PPUiTM Sungai Buloh. The Department of Patient Information now operates in only two locations, namely HASA Puncak Alam and PPUiTM Sungai Buloh.

Quality Objective


                    I. To ensure no less than 90% of medical reports are completed within 28 working days.

 

       II. To ensure no less than 95% of medical records that were despatched from wards to the Patient InformationDepartment  within 72 hours of discharge.

 

                III. To ensure no less than 90% of diagnoses are coded before the 15th of the following months.

 

           IV. To ensure no less than 85% of patient records for appointment traced at least one day prior to the date of appointment.
