Proof of U.S. Citizenship

 Must be a birth certificate, Consular Report of Birth Abroad or Certificate of Birth, Certificate of Naturalization or a previously issued full-validity passport book or card.

IMPORTANT NOTICE: All customers applying for a passport using form DS-11 must submit a clear copy of their citizenship evidence in addition to their original or certified copy of U.S. citizenship. Birth abstracts from California and Texas are not acceptable. The long form Birth Certificate is an acceptable proof of U.S. citizenship.

All documentation submitted as citizenship evidence will be returned to you. These documents will be delivered with your newly issued U.S. passport or in a separate mailing.

The U.S. Passport Card can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry and is more convenient and less expensive than a passport book. The passport card cannot be used for international travel by air.

 Both parents must appear with a child under age 16. If one parent cannot appear, a notarized consent must be obtained from the non-appearing parent. The non-applying parent or guardian must also provide a photocopy of the front and back of his or her ID with the consent form. If the absent parent cannot be reached to give written consent, the applying parent must provide an explanation on the consent form as to why the absent parent cannot be present. If the child is over 16 years old, only one parent needs to appear.

Whether you already have a passport or not, please see below for some important things to consider! Some of the following details are specific to U.S. passports, but the essence of this advice also applies to students planning to travel with non-U.S. passports.

If you have not yet applied for a passport and are wanting to study abroad very soon, please check with a study abroad advisor to see what the timeline looks like for the program you are interested in. Some programs require you to have your passport by the program application deadline and others may require you to apply for a visa before you might get your passport back. We can advise on whether you need to select the expedited passport service, or if a program is still a good option depending on the timeline. We can also suggest other programs that may not have as tight of a deadline, if needed.

An appointment will be required to get a passport and many passport application acceptance facilities have limited hours. Additionally, appointments are being prioritized for those with emergency needs, so you may end up waiting a few weeks. Bottom line - schedule your appointment ASAP.

Routine or normal service to receive a passport is estimated at 8-11 weeks. Expedited service, which costs an additional $60, is estimated at 5-7 weeks. Yes. Many passport application acceptance facilities have limited hours, so in-person appointments (required for first-time passports, renewals or those who were under 16 for previous passport, or for replacing a lost or stolen passport) can be limited and must be scheduled in advance.

Some programs, particularly those that will require you to start applying for a visa soon after acceptance, will require you to have your valid passport by the application deadline. Other programs may just require proof you have applied for it by the deadline. In all cases, it is almost always better to apply early.

Campus Connection now has the ability to take and print photographs for your US Passports. After having your photo taken, you will receive two photos printed in compliance with the United States government regulations. Passport photos are $3 for students, and $5 for Faculty/Staff and General Public.
