5. ID of the Patient

6. ID of the Client

7. Photocopy of all requirements

8. Intake Sheet/Case Study from the Municipal Social Welfare and Development Office

STEP 1 Complete the following requirements:

1. Barangay Certification of Indigency

2. Death Certificate

3. Funeral Contract (indicating the remaining balance)

4. ID of the deceased person

5. ID of the Client

STEP 2 Seek Mayor's Approval

STEP 3 Photocopy all requirements

STEP 4 Get Intake Sheet/Case Studey from MSWD

STEP 5 Submit all documents to the Mayor's office

1. Barangay Certification of Indigency

2. Certificate of Registration/Enrollment

3. Valid Issued School ID of the Student

4. Copy of previous grades
