• A Place for Mom – Nation-wide free service, connecting families to senior living options.
• AgeWise Eldercare Solutions – Free service to access elder healthcare resources.
• Senior Resources – for remaining or returning home, home assessment, Medicaid waver pgm.
• Elder Helpers – Free help carrying groceries, running errands, writing letters, cleaning, cooking etc.
• Advanced Directives in Michigan – information about Patient Advocate Designation form for Michigan.
• Social Security Administration – information about benefits for people with disabilities.
• Social Security Administration – information and application for retirement benefits for self or spouse.
• South Bay YMCA, Holland – Rec and community services activities.
• Ottawa County Loan Programs – Hospital beds, scooters, lift chairs, walkers, canes, shower seats etc. as long as needed.
• MY MONEY.GOV – Prepare financially for retirement.
• Medical Alert Advice – 3 fall alert devices reviewed, AND 7 important questions for an aging parent.
• Fall detection devices – Monitors, bracelets, watches, glasses and prevention.
• HOME CARE by RESTHAVEN – Holland – Helps create a plan to help people safely stay in their own homes or to find an appropriate alternative.
• Lakeside Vista – Holland – Assisted Living, Nursing Home, Independent Living, Hospice, memory care, transportation svs., Medicaid and more.
• Long Term Care Ombudsman – Ottawa County – Advocacy for residents in LTC facilities, and explanation of rights.
• Ottawa Care Center at Heartwood Lodge – Spring Lake/Ferrysburg – Residential and hospice care for those with a terminal illness.
• Adult Protective Services – Protection for vulnerable, endangered adults. Independent living. Eligibility.
• Providence Life Service – Zeeland – Long term, skilled & basic nursing health care & Alzheimer’s care. Rehab services, physical, speech and occupational therapy etc. “Best Nursing Home 2019-20 US News & World Report.”
• Res Care Home Care – Holland – Alzheimer’s care – Hospital to Home – Caregiver care – Nursing care. Medicaid waiver pgm. if one qualifies financially and by condition.
• Riverside Nursing and Rehab Community – Grand Haven – 34 Bed, short term rehab. to respite care to hospice to long term care. 2015 Best nursing home designation by US News & World Report. Medicare and Medicaid.
• Meals on Wheels Western Michigan – Sign up site with information.
• Waverly Meadows – Holland – Senior apartments convenient to grocery stores, banks and restaurants. Low income housing tax credit.
