Steps to apply for California Children's Services (CCS):
Check if your child might qualify
CCS serves children with eligible physical or medical conditions such as:
Cerebral palsy
Spina bifida
Neuromuscular disorders
Chronic orthopedic issues
Certain genetic or neurological conditions
Must be under 21 years old and a California resident
Get a medical diagnosis
A doctor must diagnose your child with a qualifying medical condition
You'll need documentation from your pediatrician or specialist
Complete the application
Fill out a CCS application form (available online or from your local CCS office)
Submit the application
Submit the completed form and documents to your county CCS office
