The OOC Endometriosis Excision fund begins at 9:00 AM EST on January 15, 2021 and ends at 11:59 PM EST on June 15, 2021. By participating, nominators/nominees agree to be bound by the Official Rules and the decisions of the panel.

Eligibility
Nominee has either diagnosed or suspected endometriosis. Any age. Located anywhere globally. Nominators can nominate themselves or another individual. It's recommended that nominators have permission from the nominee to nominate them, and some of their medical documentation to include in the application.
Nominees will be deemed eligible based on the above and applications weighed by the severity of case, financial situation, and current quality of life.
All applications must be complete and include all required information.
In order to qualify, a written explanation or video of the current experiences of the applicant is needed. In addition, a consultation letter for the surgery (can be provided post-application) and medical documentation (must be provided at the time of application, more may be asked for if needed) must be provided.
Nominators may nominate more than one person. Only one application per nominee is allowed (nominators can nominate multiple people in their lives that have endometriosis, however they cannot submit duplicate applications when nominating someone)
Selection
All applications will be reviewed by the organizer. Final decisions will be made by the entire panel (organizer and doctors listed) to choose up to five recipients, contingent on the amount raised through Gofundme. Applications will be reviewed and recipients chosen by the first week of September 2021. If $20K CAD is fundraised before September 2021, the first applicant(s) will be notified. The number of applicants is dependent on how much is fundraised. Due to a high volume of applications, recipients may not be notified if their application is not chosen. If chosen, they'll be invited to interview to learn more about their current situation. They may also be required to send in additional medical documentation. After the interview, the five recipients will be notified via email if they're chosen. Based on the submission of documentation, a consultation letter, an explanation of current experiences, and an interview, a recipient will be chosen. An alternate recipient may be chosen in the event that a recipient (1) cannot be reached within 5 business days (2) declines the fund (3) refuses to submit the required documentation or share their story through media platforms.
Funds
In the event that less than $100K CAD is raised through Gofundme, the number of recipients will depend upon the total amount raised (ex. if $60K is raised, only three recipients will be chosen). The fundraiser through Gofundme may run until the end of 2021 if required to raise at least $20K CAD.
FemEvolve Inc./Christina Paruag (The Organizer) will not be held responsible for any complications that arise from surgery, dissatisfaction of surgery by the recipient, or additional fees required for surgery. FemEvolve Inc./Christina Paruag (The Organizer) will not be held responsible if funds are not disbursed by April 2021 as the fund is contingent on how much is raised through Gofundme. A maximum of $20K CAD will be given and additional costs are the responsibility of the recipient.
Media
By applying, nominee agrees to having their before AND after surgery stories shared through FemEvolve, Christina Paruag's social media platforms and various media platforms, either with name/picture or completely anonymously. Nominee can agree once notified they've been chosen as the nominator cannot make this decision for nominee.
