Group registration Form

CLASS SELECTION FORM (GROUPS ONLY)

Click on the above link to access a special class selection form for groups of 10 or more dancers.
You will be prompted to make a copy of the form.
Complete the form by including each dancer's name and their class selections. A drop-down menu will show each class that is offered.
If the dancer is participating in a Performance Evaluation or taking a break, leave the column empty.
Once you have completed the form, Save it. You will upload the file to your Group Registration Form (see the link below).

GROUP REGISTRATION FORM

For Groups of 10 dancers or more

Once this form is submitted, an invoice will be sent to the contact email address provided. Payment is due within 14 days of the invoice date and no later than November 13, 2026.

Payment options:

Mail a check, payable to Farmington Public Schools c/o Toi Banks, 32000 Shiawassee Rd, Farmington, MI 48336

Online Payment can be made with a check or credit card. No additional fees will be assessed for check payments using this payment option. Please include 3.6% fee for credit card payment.

Registration fee: $65 per dancer (no refunds)

College Showcase (1 per person)

Scholarship Audition (for eligible dancers)
