Jackie Ward was an incredible Mother, Athlete, Cook and Best Friend to many. Like many people she was diagnosed with dementia. In June of 2024 she was admitted to Oak Park Place Memory Care (Autumn Lane) in Baraboo, Wisconsin. Jackie was so independent all her life, this transition was extremely rough. She eventually settled in and was visited by her family almost daily. (scroll down)
In June of 2025 Oak Park Director of Health Care Services advised that Jackie should be admitted to hospice due to several falls. The Director pushed a certain Hospice company but ultimately Jackie was accepted by the non-recommended hospice agency, Agrace Hospice. Agrace was wonderful and was communicative with Jackie's family.
On July 10, 2025 Jackie fell out of her chair and an Agrace Nurse came to visit and assess her. The Agrace nurse attempted to move Jackie back to her bed and requested help from the Oak Park Staff. The Oak Park Place staff was un-cooperative and subsequently the Director of Health Care Services directed the staff to tell the Agrace nurse to leave the premises. The Agrace Nurse was "kicked out" and denied help in returning Jackie to her bed.
Jackie's family have reason to believe that Jackie was kept in a recliner or wheel chair and not returned to her bed for three weeks or longer.
On July 24 , Jackie was moved to the Hospice House in Baraboo. Jackie was so happy to move and the staff and facilities at the Hospice House were simply amazing. During the initial physical at check-in, it was determined that Jackie had a pressure sore on her posterior. No doubt from being in recliners and wheel chairs.
Jackie found peace and calmness at the Hospice House. She passed on August 15 with family by her side.
