HOW TO AVAIL HMO SERVICES AT NOVAGEN HOSPITAL: 

1. At the ground floor of NovaGen Hospital, proceed to the HMO Service desk beside the reception area. 

2. Present your HMO card to the agent for assistance.

3. Along with your valid ID, present your accomplished Request Procedure and your LOA form. The request procedure form will be provided and filled out by your physician.

4. NovaGen will contact your HMO company and verify your personal details, procedure/s requested and filed LOA. 

5. Once verified, your HMO company will provide an LOA approval code. Afterwards, NovaGen will register your details and your procedure.
