Please fill out this form if you are interested in using it for this year's NSYA list. We will send you additional information and a link to sign in for you and your readers when it's complete.
Students who read 3 or more NSYA books are eligible to vote for their favorite book at their school or public library.

We request that votes be submitted in one batch from each school or library. We encourage you to collect your votes by creating your own voting form tailored to your students. A printable voting form for teachers & librarians is available for download at the bottom of this page.
