COURSE OBJECTIVES
This module is designed to help health center staff understand best practices for prioritizing what matters to older patients and their caregivers in determining end-of-life preferences.
After completing this module, participants should be able to:Ā
Understand the goal of advance care planning and the value of evolving conversations about patient preferences
Describe the purpose of healthcare proxies and the process of appointing a proxy in accordance with state laws
Utilize shared strategies for engaging older patients in advance care planning conversations and addressing related concernsĀ Ā
This module has four sections including: Section 1: Understanding Advance Care Planning, Section 2: Advance Care Planning is a Process Over Time, Section 3: Communication Basics for Advance Care Planning, and Section 4: Naming a Health Care Agent. To receive a Certificate of Completion for this module, you must complete the Knowledge Check assessments after each section and submit the final evaluation form to verify your participation.
