COURSE OBJECTIVES
This module is designed to help health center staff understand key components of welcoming and livable communities that support the ability of older adults to safely age in place.
After completing this module, participants should be able to:
Understand how improving the livability of communities for older adults benefits residents of all ages
Describe the interconnected domains of age-friendly communities that support older adult health and well-being, including access to healthcare
Consider evidence-based approaches for adapting programs and policies to meet the needs of an aging patient population
This module has four sections including: Section 1: What does it mean to be "Age-Friendly"?, Section 2: What do Older Adults Need to "Age in Place"?, Section 3: Making Healthcare Age-Friendly, and Section 4: Community Efforts.
