Prevalent Medical Conditions

PREVALENT MEDICAL CONDITIONS

If you have a child with a prevalent medical condition:

• Anaphylaxis

• Asthma

• Diabetes

• Epilepsy/Seizure

•  and/or others we want to make sure the school has a current Plan of Care to support your child. 

All parents of children with prevalent medical conditions are asked to complete a Plan of Care and submit it to the school at the start of each school year. 

Any changes to a child’s Plan of Care throughout the school year should be promptly communicated to the school.
