Electronic Health Records is defined as a digital record of the patient's longitudinal personal health related information that is accessed, entered, stored, retrieved, exchanged, and evaluated as a result of information generated for patient care encounters (or visits) within or across health care organizations(Hammond, 2005, p. 38).
Electronic Health Records or EHR is a systematic and organized way to store all the patient related information electronically. It's a life time record of personal health care information which will be accessible to that individual, health care professionals and health care organization.
A brief information about Electronic Health Records.
Components/Objectives of Electronic Health Records:
Health data and information: The main database of EHR is health data and information of an individual.
Oder entry and management: It includes the entry of care and treatment provided by respective health care professional.
Administrative processes: All the administration related patient information such as demographic, insurance etc.
Clinical decision support: A supportive information and knowledge of an individual to all health care providers, also it can be active or passive.
Electronic communication and connectivity: An Electronic mode of communication between health team with the use of internet connectivity.
Results management: It incorporates the results from tests performed in clinical areas, other diagnostic test and consultative exams.
Patient support: Providing health information to the patient about their diagnosis, treatment and care.
Reporting and population health management: Able to access the data on EHR by using certain criteria such as age, date of admission etc.
Types of Electronic Health Record Soft wares:
1. Physician-Hosted Software: In this type of EHR system, the Physician will be the responsible person for purchasing the hardware and software as well as maintenance and the security of the data stored.
2. Remotely-Hosted Software: In this type of EHR system, the storage of data is shifted from the physician to a third party. And that particular third party will be responsible for the maintenance, data backups and security.
3. Remote Software: In this type of EHR system, there are three different varieties which are subsidized, dedicated and cloud respectively.
