β Fill in your name, address, and contact information accurately.
π΅ Section C - Track
β C1 - Tick YES to the Small Claims Track (This is for claims up to Β£10,000 β limited costs apply).
π΅ Section D - Attendance at Hearing
β D1 - Tick:
YES if you do not want to attend a hearing (this is rare).
NO if you want to attend and cross-examine the Claimant's witness.
π If you tick NO (you want to attend), add this note:
"The Defendant respectfully requests the opportunity to cross-examine any witness relied on by the Claimant."
π΅ Section E - Telephone Mediation Availability
β E1 - Enter your Phone Number (for mediation contact).
β E2 - Are you unavailable for mediation in the next 3 months? Tick YES or NO.
If YES, list any dates you cannot attend.Β
β E3 - Do you need an interpreter for mediation? Tick YES or NO.
If YES, state the language you need.Β
β E4 - Are you vulnerable? Tick YES or NO.
If YES, explain any vulnerabilities that could affect the mediation process (only list those relevant to mediation).
π΅ Section F - Hearing Arrangements
β F1 - Hearing venue: Write your local County Court.
β F2 - Tick NO to expert evidence (this is for medical/legal expert reports β not relevant here).
β F3 - How many witnesses? Put 1 (thatβs you) unless you have someone else giving a statement.
β F4 - Any pre-booked holidays? List dates you are unavailable.Β
β F4 - Interpreter needed? Tick NO (unless you need one for the hearing itself).
β F5 - Are you or your witnesses vulnerable? If yes, provide details.
π΅ Section G - Language
β G1 - Have you been told you can give evidence in Welsh? Tick YES.
β G2 - Preferred language: Tick ENGLISH (unless you prefer Welsh).
β G3 - Documents language: Tick ENGLISH (unless you want documents in Welsh).
