Black Elk Elementary Student Meal Order Form

Please fill out this form to order an Elementary meal for curbside pick up. Orders must be submitted by 10:00 am on the day of service for pick up. You will receive one hot lunch and a cold breakfast for the following day. You must submit an order form for each day you would like to pick up a meals for your students.
