Additional forms required for neighborhood schools:

MPG sites that are interested in student participation for comprehensive/neighborhood school sports, and are NOT registered need to complete the Membership Application - Association Form

Here's the list of MPG sites that are currently registered under OSAA. If your site is not listed, you have to complete the Membership - Application - Association Form for your student(s), and pay a $250 yearly fee for your students to participate at their neighborhood school.

Additionally, each site should have a School Representation Eligibility Certificate completed and stored on-site for student-athletes participating at their neighborhood school athletics.
