(M-C.A.R.D.)
A safer, smarter way to protect our most vulnerable.
What is the Monterey Community At-Risk Database?
The Monterey Community At-Risk Database is a vital local initiative designed to reduce deaths and injuries among at-risk individuals, including those with dementia, autism, and other Intellectual and Developmental Disabilities (IDD). By building a strong connection between first responders and the IDD community, M-C.A.R.D. ensures faster, more informed, and compassionate responses in critical situations.
What does the M-C.A.R.D. provide?
An information database to better understand and support the local IDD population
Individualized needs assessments and service planning for first responders
Tools for de-escalation and communication to ensure safety and dignity
Promotes fairness and reduces misinterpretation or bias
Active collaboration with caregivers and guardians for informed care

If she were to go missing, it’s critical that first responders know key facts immediately: her address, emergency contacts, her attraction to water despite not being able to swim, and that she is autistic and non-verbal.

How do I enter someone into the database?
There's a few different ways to enter someone into the M-C.A.R.D. First off, the individual giving the information needs to be authorized to give the individual's information on their behalf (e.g. parent, guardian, caregiver, etc.)
-ONLINE: If you have a Google/Gmail account, click this link to fill out and submit an online questionnaire: Online Form
-PAPER FORM: If you would prefer to download and print out a paper form to fill out, click this link: Paper Form
Once the form is complete, you can scan and send it to montereypolicetn.gov@gmail.com or drop it off at the Police Department Monday-Friday 8:00 am to 4:00 pm.
-IN-PERSON: Or, you can simply stop by the Police Department to pick a form up. Fill it out there and leave it with us, or complete it and get it back to us at your convenience.
