Booking an appointment

• Everyone must have a ‘request form’ from their treating doctor. Monash Health accepts other imaging provider request forms as well.

• To contact us, visit locations, services and opening hours for phone numbers and email address for each of our sites.

• Please ask our reception staff for appointment times.

• You can make an appointment either by phone, email, fax or visit us.

• If you would like an interpreter for your appointment please ask our receptionist to book this for you.

• Make sure your mobile phone number, postal address and email address are correct on the referral.

• Monash Health Imaging sends an SMS reminder to your mobile phone a few days before your appointment date. You may also receive a phone call if you do not reply to the SMS message.

• During the booking procedure you will receive detailed instructions and explanation about the preparation required for your imaging test. Instructions can also be posted or emailed to you.
