To Donate via Credit Card: Please visit our online donation platform.
To Donate via Check: Please complete this form and place your donation in the provided envelope. Students can give envelope to their 2nd period teacher or bring it directly to the office. Cash is discouraged. CUSD provides all necessary supplies for student learning. Donations are voluntary and no child will be denied participation. Please make sure checks are written to "Miller Middle School PTA" and include your child's full name and grade level on the check.

Employer Matching
If you made your donation through our CEEF platform, you will need to donate to CEEF, instead of Miller PTA, when submitting your matching request. Your donation still goes to Miller PTA - CEEF acts only as a pass-through entity and transfers all matching funds they collect on our behalf to Miller PTA.
How do I submit the matching request?
Once you have logged in using your company credentials to your Employer Matching portal, donate to ‘Cupertino Educational Endowment’. In the comments, please be sure to include our school name (Miller Middle School PTA) and use CEEF's EIN number (77-0073617). This will ensure the match is transferred to Miller PTA.
What if I donated via Check?
The employer matching process for Check donations has not changed. After you've logged in to your Employer Matching portal, please donate to 'Joaquin Miller Middle School PTA', EIN number 94-6184025.
