Handout - MMS Spring Meeting 2026 Season
Physical Information: Your runner will need a physical to participate in cross country for the fall 2026 season. If you visit your child's Sapphire account, you will see a link to view his/her Athletic Form which allows you to verify/update your child's personal history/health information. There is also a link to the physical form that needs to be completed for your child to participate in a fall sport. Please print out this physical form and take it with when you go for a physical appointment or well visit. Please see the link below from the district's website for physical information. (This is similar information to what you will find on your child's Sapphire account but you still have to visit your child's Sapphire account to fill out his/her Athletic form and update his/her health history.)
