Transition Mission

At Mount Horeb High School, our transition mission for our students is to empower them to be active members of their community, prepare them for a successful future, and develop their leadership potential. We believe that all students deserve the opportunity to learn, grow, and lead, regardless of their abilities.

Our focus is on providing students with the skills, knowledge, and resources they need to succeed in post-secondary education, employment, and independent living. We believe that by working together with families, educators, and community partners, we can help students achieve their full potential.

Our transition mission is centered on the following principles:

COMMUNITY: We believe in fostering positive relationships and connections within the school and wider community. We encourage students to be active and engaged citizens who contribute to their communities in meaningful ways.

FUTURE: We are committed to helping students prepare for a successful future. We provide them with the tools they need to pursue post-secondary education, employment, and independent living.

LEADING: We believe in developing students' leadership potential. We encourage them to take initiative, advocate for themselves and others, and become confident and effective leaders.

PREPARE: We are dedicated to preparing students for the challenges and opportunities they will face in the future. We provide them with a comprehensive curriculum that emphasizes academic, vocational, and life skills.

LEARN: We believe in creating a supportive learning environment that promotes student growth and development. We use evidence-based practices and individualized instruction to meet each student's unique needs.

LEAD: We are committed to supporting students as they take charge of their own learning and become leaders in their own right. We encourage them to set goals, make plans, and take action to achieve their dreams.

Mission Statement and Objectives (Revised in 2020): Mission: To share the work, establishing effective communication, and fostering new relationships to better work together to improve transition from high school to college or career for students with disabilities. Goal: Enhance the knowledge of post high school options for students with disabilities and their families. -Objectives: • Looking Forward transition night event Goal: Promote better connections for HS staff and providers of adult services. -Objectives: • Host 9 monthly meetings with staff and adult providers • Invite more adult providers to attend monthly meetings. • Invite adult providers to speak at specific meetings about their services.
