Task 1:  Creating a medical questionnaire form

What is a Medical Questionnaire? 

This is a set of written questions asked during the application process. It is a document that patients use to provide details of their medical history such as pre-existing or chronic condition details.

What needs to be done

• You need to create a form to collect information about the patient's health.

• To create a form, use the Google Forms service.

• To open the service, click the "Start forms" button located below on this page and log in to your Google account.

• Create a new form and add at least 25 questions to it.

• You can use the questions from the examples provided on this page or come up with your own.

• You can copy and paste the questions from the example into your new form.

• Select the correct question type: "Date" for the current date and date of birth, "Short answer" for name, phone, etc., "Checkboxes" for questions with options, "Multiple choice" for yes/no answers.

• After creating the form, you must fill out the check-form at the bottom of this page.

• Enter your full name in English in this form, select a group, task number, and a link to your form. To get a link, see the instructions below.

Google Forms – along with Docs, Sheets, and Slides – is part of Google's online apps suite of tools and an effective computerized questionnaire system. It's easy to use and one of the simplest ways to save data directly to a spreadsheet, and it's the best sidekick to Google Sheets' spreadsheets.

Google Forms is paper based questionnaires were programmed to allow a simple to read and complete push button format. A confidential log in system was developed that allowed secure data collection. No personal identifying information was collected, although additional productions could be allowed to do this. The responses were formatted so all questionnaire data could be copied to separate spreadsheets for subsequent data analysis.

Note: Google Forms service works correctly in Mozilla Firefox or Chrome browser.  It's better to use a browser Mozilla Firefox.

Сreate form like an example. Your form must contain some important questions about patient health status - temperature, blood pressure, pains etc.

You need to send a link to your form  for check  by the teacher.
