Our clinic restricts our treatment panel to a limited number of pre-qualified patients and our Clinic accepts only patients who are serious about overcoming opioid addiction. So dangerous or inappropriate behavior that is disruptive to our clinic or to other patients may result in your discharge from the Suboxone treatment clinic
• Missing appointments. Don't miss your appointment. If you don't show up in 2 months then you will need to re-register as a new patient
• Running out of medication too soon & requesting early refills. Your prescription should last 30 days if taken as prescribed. Refill can only be done on the 29th day after your last appointment. Mark it in your calendar. Click here for appointment or text 4046894449
*Combining Suboxone with opioids
• Agitated behavior. With Suboxone the customer is not always right. The DEA is always right. Suboxone is a controlled substance. We follow DEA rules strictly
• Frequent or urgent inappropriate phone calls or Text
• Outbursts of anger
• Lost or stolen medication. if you medication is lost or stolen we will need a police report to refill your medication early
• Credit card fraud; This is serious and will be reported to the police immediately. DO NOT USE another person's credit card to book an appointment without their consent. If you use your credit card DO NOT CALL your bank to deny any knowledge of the transaction.
Sending medication out of State :Your medication can only be sent to the state you live in. If you live in Georgia DO NOT request that your medication be sent to a pharmacy in another state. if you are going to See Uncle XYZ who is in coma. in Arizona, get your medications here before leaving or Aunty XYZ whose wedding you can not miss. Get your medication before traveling out of state.
* Request to Change pharmacy or request to change From strips to tablets or tablets to strips MUST be sent via text to the clinic number 4046894449 no later than 6AM of the day of your appointment. If we don't get any request from you before 6AM your medication will be sent to the last pharmacy we have on file. Medication CAN NOT be changed after the doctor has sent it to your pharmacy.
* To avoid delay getting your medication, Please Call your pharmacy ahead of time to make sure they have the medication in stock before your appointment day.
We DO NOT assume general medical care of Suboxone patients. Your primary care doctor should treat your other medical Problems. we only treat opioid addiction and nothing else.
.We don't accept any insurance. All Patients must adhere to strict cash payment policies. Making a payment and later calling your bank to say you have no knowledge of the transaction is a crime in Georgia called theft of Service and will result in getting the police involved.
