These forms allow you to certify to one of the CDL categories listed below, which determines if a medical variance or a U.S. Department of Transportation (DOT) Medical Examiner's Certificate is required to be on file with the Department.

Category 1: Non-Excepted-Interstate Commerce (CDL-4)

Most CDL holders who drive CMV's in interstate commerce are non-excepted interstate commerce drivers. This category requires a medical examiners certificate.

If you operate in both excepted and non-excepted interstate commerce, you must follow non-excepted interstate commerce guidelines and maintain a current medical examiners certificate with the Department.

In certain instances, the Federal Motor Carrier Safety Administration may provide an exemption to specific medical conditions. For more information, see Driver Exemptions Programs. If you have been granted a federal waiver, you must include it with your medical examiner's certificate.

Category 3: Non-Excepted-Intrastate Commerce (CDL-5, Section B)

This certification category is required when you are only driving a CMV in intrastate commerce and are required to meet the medical requirements for Texas. A medical examiners certificate will be required. 

In certain instances, the Department may provide an exemption to specific medical conditions when operating in intrastate commerce. For more information, please review form CDL-37. If you have been granted a state waiver, you must include it with your medical examiner's certificate.

Category 4: Excepted-Intrastate Commerce (CDL-5 part A)

This certification category is required when you are operating a CMV only in intrastate commerce. You certify that you have been regularly employed operating a CMV in Texas prior to August 28, 1989 and are not operating a CMV requiring a hazardous materials placard. This category will NOT require a medical examiners certificate, but you are still required to meet Texas medical standards.

Category 4: Excepted-Intrastate Commerce (CDL-5 part C)

This certification category is when you are operating a CMV only in intrastate commerce for excepted activities (oil/water well servicing and/or drilling or mobile crane operations). You will be restricted to operate your CMV in the excepted activity to which you have certified. This category will NOT require a medical examiners certificate, but you are required to meet Texas medical standards.

If you are certified to a non-excepted medical status and have a medical examiner's certificate on file with the Department that is going to expire, you can send your updated certificate to the Department via email, fax, or mail. Please do not send duplicate documents unless you have been instructed to resend due to a problem or issue with the document you previously submitted. You may send in your medical examiners certificate for processing only if your CDL or CLP has not been downgraded.

If your CDL or CLP has been downgraded due to an expired Medical Examiner's Certificate, you must visit your local driver license office to be upgraded. A CDL that was downgraded to a driver license (DL) solely for failure to provide a valid medical variance or medical examiners certificate, may be upgraded back to your previous CDL status (including restrictions and endorsements) without taking CDL knowledge and skills exams provided the following conditions are met:

CLP holders will have their permits cancelled if an updated Medical Examiners Certificate (MCSA-5876) is not submitted and added to the drivers record within 55 days of the letter date. Employers that subscribe to the NY State License Event Notification Service (LENS) can sign up to receive notification of these status changes. Passenger carriers enrolled in the 19-A system will also receive status change notifications.

If you have a commercial driver license, Class A, B or C or CDL permit issued by the NY State DMV you must certify the type of driving you do anytime you perform a license transaction (other than an address change, or duplicate license). You will be required to choose only one driving type from the four federal driving types listed below.

All healthcare professionals can apply to become a certified medical examiner if their scope of practice authorizes them to perform physical examinations, as defined by the state in which they practice.

Q. What must I do to comply with the new requirements for making my medical certification part of my CDL driving record?

A. Starting on January 30, 2012, when you:

Federal Motor Carrier Safety Administration (FMCSA) regulations 49 C.F.R. 383.73 and New Jersey regulations N.J.A.C. 13:21-23.28(l) require all Commercial Driver License (CDL) holders to submit a CDL Holder Self-Certification form ( view sample) and a valid (non-expired) Medical Examiner Certificate (MEC), if required. If a federal medical variance has been issued, a CDL holder will need to submit a copy of the variance along with a MEC to the Motor Vehicle Commission (MVC). The CDSC-1 CDL Holder Self-Certification form is available for download or at a motor vehicle agency .

Yes, you will be downgraded until such time that you provide the new medical certificate to the CDL Office at the Cranston DMV. This can be done by either scheduling an in-person reservation or by using the online service for medical certificates on the DMV Website

Upon completion of entering all the medical certificates received through our online service, an email will be sent to inform you that it was processed successfully. If for some reason we were not able to process the medical certificate submitted, an email will be sent to explain why it was not successfully processed.

If you did not submit the proper format or if a mistake was made on your email address, you will not receive confirmation. Please be sure that your email address is correct before submitting. The medical examiner will fill out a Medical Examination Report (long form). Upon passing the physical examination, the medical examiner will provide the driver with a copy of a medical certificate. The motor carrier and driver are responsible for having copies of the current medical certificate.

Drivers must be examined at least every two years; medical examiners can issue a certification for less time at their discretion. If the driver returns from an illness or injury that may interfere with driving ability, he or she must undergo another medical examination, even if the medical examination certificate has not expired. Federal exemptions for visual and diabetes must be renewed annually.

Note: If the Federal Motor Carrier Safety Administration has given you a Skills Performance Evaluation Certificate and/or a medical exemption or Oregon DMV has given you a state waiver, you must carry the original or copy of that medical variance documentation at all times when on-duty.

We review the DOT Medical Examination Report (long form) and Medical Examiner Certificate (cert card, or short form) for completeness and ensure the medical examiner has qualified the driver according to Federal Motor Carrier Safety Administration (FMCSA) Regulations.

Arizona requires all CDL drivers, including those in the military, to keep a current Medical Examiner Certificate on file with MVD. The Medical Examiner Certificate is to be renewed every 24 months or sooner, as determined by the medical examiner or by regulation. Failure to maintain a current certificate on file with MVD will result in the suspension and revocation of your CDL.

If you receive a request for additional information this means your medical certification was incomplete, you require restrictions to be added to your license, or both. Please note that only the boxes marked apply to you. Corrections to your examination report must be initialed and dated by your physician.

All CDL holders required to have a valid medical certificate must comply with the regulations in Part 383. The regulations provide no exceptions to service members on active deployment. If a service member can provide an acceptable medical certificate, his/her medical certification will continue. Otherwise, the state will have to begin the downgrade process.

CDL drivers who certify to either a "non-excepted interstate" or "non-excepted intrastate" category of commercial motor vehicle (CMV) operation must have their physical examiner's certificate completed by a medical examiner who is listed on the Federal Motor Carrier Safety Administration's (FMCSA's) National Registry of Medical Examiners.

The Department of Motor Vehicles (DMV) will not accept your medical examiner's certificate unless it was completed by a medical examiner listed on the National Registry of Medical Examiners and the certificate displays the medical examiner's registry number. 17dc91bb1f
