You have 3 choices to complete the form: manual print & sign, web interactive form or digital certficate signature. Players and parents can not begin or attend any services on court without Medical and Liability Waiver for each person that will be on court (without a waiver, please remain outside the court area). If we have a previous submittal on file from a recent past John Hebert Tennis or John Hebert Pickleball provided service, you don't need to submit another. Please also review tennis and club rules and etiquette given far below.
