A prepaid healthcard which provides one-time Confinement coverage for eight (8) identified illnesses

Maxicare PRIMA is an affordable prepaid healthcare plan for individuals which offers unlimited Outpatient Consultations, and availment of 255 types of laboratory and diagnostic procedures within Maxicare’s network of Primary Care Centers (PCC) and MyHealth Clinics.

2. Enter the Reference Number that you’ll receive by email after purchase
3. Provide the required personal details and click “Submit”
4. Once successfully registered, you will receive an SMS notification and an email together with your electronic card (eCard) within 24 hours
5. Your membership will be activated 7 days after successful registration

3. Once successfully registered, you will receive an SMS notification with your Policy & Card Number and an email notification together with your electronic card (eCard) within 24 hours
4. Your membership will be activated 7 days after successful registration

• Present your Maxicare EReady card or eCard along with 1 valid ID with photo upon availing the services offered in this healthcare plan
• No card or eCard, no availment
• Membership is valid up to 1 year from the date of activation
• You can use more than one card in a year but only one can be active at a time
• The card is non-transferrable once registered

• Subject to 15-day free-look period, but you may only refund the product if you haven’t used the card
• There will be no maximum number of cards for registration but member can only register another card after 3 months from the date of availment of first registered card

   • alcoholic liquor intake
   • unauthorized use of prohibited drugs or regulated drugs

   • war or any combat-related activities while in military service,
   • extreme sports,

   • circumcision and sex transformation,

   • attempted suicide,
   • acupuncture,
   • congenital, genetic and hereditary conditions,
   • chronic dermatoses
3. HIV and AIDS-related diseases, sexually transmitted diseases
4. All pregnancy and maternity-related conditions
5. Epidemic or pandemic infectious diseases
6. Psychiatric and/or psychological disorders
7. Aesthetic, cosmetic and reconstructive surgery
8. Oral surgery and dental treatments
9. All types of vaccines except for animal bites
