Participants in any Scouting event must complete Parts A and B annually. They contain a health history, parental/guardian informed consent and hold harmless/release agreement and a talent release statement. Please attach a photocopy of both sides of the insurance card. The pack is required to have medical forms on hand for all participants for events like Winter Blast, Summer Camp, River Rats, and any pack outing.
Pro Tip! Save the electronic copy to your computer so you can update it yearly without needing to start over.

The safety of our Scouts is our top priority. We ask that all parents/guardians take S.A.F.E. Guarding Youth training online. This is very similar to the trainings required by local sports leagues. It only takes about an hour online, and helps ensure that every single adult is aware of the safety expectations throughout Cub Scouts and Scouting America. You do not have to be a registered member of Scouting America to take Youth Protection training. Together, we can ensure that every Scout experiences a program that is safe, respectful, and fun.
