Occupational Therapy comes in about two times a day to help Luca sit up.

Sept. 26 - Today Luca was much more coherent!

Sept. 27 - Luca stood up! With OT/PT help. Next step is walking! Luckily his pelvis, leg, and knee fractures are all weight bearing. Due to all his upper body injuries, they have a belt that they tie around his waist to help hold him up.

Sept. 28 - We snuck Moose, the white dog, into the hospital to cheer up our bro. 

Today, Luca said "If I do well in PT, I think I deserve a milkshake!" Sadly, he is still getting his nutrition through a feeding tube, so no shake yet.

Physical Therapy are working to get him walking.

Sept. 29 - Luca took a little trip around the hospital pond today. 

The past two days Luca has made leaps and bounds, but is still really out of it. He has short term memory and data retrieval struggles. 

Doctors say that progress will most likely be seen, not within days, but months.

Oct 31 - Luca is slowly turning back into a human guy...even though he sometimes looks a bit like Frankenstein with all the scars and metal in the body!

Luckily, Luca had the pins in his left fingers taken out on Halloween. It is not fun or particularly pain free, but he has handled it all like a champ. Above is the picture of the size of the pins contrasted to his right hand. They were also able to remove the rest of the cast on his left wrist and elbow.

He is quite sad with how much his muscles have atrophied (gotten smaller) since the accident, and that he had to spend Halloween in the hospital still. News is that he may leave Friday?

🍕 31 ottobre - Luca sta lentamente tornando ad essere un ragazzo normale... anche se a volte sembra un po' come Frankenstein con tutte le cicatrici e il metallo nel corpo!

Per fortuna, Luca ha tolto i perni dalle dita della mano sinistra a Halloween. Non è stato divertente né particolarmente indolore, ma ha affrontato tutto come un campione. Nella foto sopra puoi vedere le dimensioni dei perni rispetto alla sua mano destra. Sono stati anche in grado di rimuovere il resto del gesso dal suo polso e dal gomito sinistro.

È piuttosto triste per lui vedere quanto si siano atrofizzati i suoi muscoli (siano diventati più piccoli) dall'incidente, e il fatto di dover trascorrere ancora Halloween in ospedale non lo ha reso molto felice. Le notizie dicono che potrebbe uscire venerdì?

Oct 26 - May have spoken too soon...

After one day at home, my dad noticed Luca's scar along his side had split open...so he took some pictures to send to doctors and they had Luca come back in to check for infection.

Although there didn't seem to be any swelling, redness, or pain, they had to surgically operate on the area to ensure no infection was within and to close it back up. Sadly, Luca has to stay at UofU hospital again...waiting for this to heal properly. There was no infection, but they had to really stretch his skin to properly close the wound. I rather not gross you all out with pictures of his split side or scars, hopefully the one above isn't too bad. It shows him after the procedure with a patch covering the area.

🍕 24 ottobre - Forse ho parlato troppo presto...

Dopo un giorno a casa, mio padre ha notato che la cicatrice di Luca lungo il fianco si era aperta... così ha scattato alcune foto da inviare ai medici e hanno fatto tornare Luca per un controllo dell'infezione. Nonostante non ci fosse alcun gonfiore, rossore o dolore, hanno dovuto operare chirurgicamente l'area per assicurarsi che non vi fosse alcuna infezione all'interno e per chiuderla nuovamente. Purtroppo, Luca deve rimanere di nuovo all'ospedale dell'Università di Utah... aspettando che la ferita si rimargini correttamente. Non c'era infezione, ma hanno dovuto stirare molto la sua pelle per chiudere adeguatamente la ferita. Preferisco non disgustarvi mostrandovi foto del suo fianco spaccato o delle cicatrici, spero che quella sopra non sia troppo brutta. Lo mostra dopo l'intervento con una toppa che copre l'area.

Oct 24 - After 6 weeks in the hospital, tonight we were able to have Luca come home! Moose (white dog) isn't the only one happy. 

We initially thought he was going to be doing in-patient rehab (in the hospital), but Luca had other ideas and was really set on coming home! Luckily, we are able to administer his medication, keep his feeding tube goin' (with his mouth still wired), and have him at home. The plan is to drive him to the hospital everyday to work at his rehabilitation and therapy. 

We feel so amazed and grateful that Luca's traumatic brain injury has not diminished his ability to function as far as we can see now. We're hopeful that the future will continue to show progress.

With rehab at the original Utah Valley hospital, the doctors also have the chance to follow-up with Luca's many fractures and soft tissue injuries. His wrist cast can be taken off, as well as the pins removed. The wired jaw will be removed within the next couple weeks *hopefully sooner*! He may even be able to remove one of the leg braces he has been wearing.

Over time we will be able to see how Luca can compensate with that right arm and hand. He will have to engage and create new neural pathways to move his arm. He does experience consistent nerve pain, but he's ready to work. He is excited for getting out of bed, a routine of therapy (OT, PT, Speech...), and seeing some progress.

Luca now has a replacement phone with the same number, so give him a call, or send a message. He greatly appreciates the thoughts and love. I know he said with all his scars that he looks like a world map...maybe that will be his Halloween costume😅 We love you all. Thank you so much.

🍕 24 ottobre - Dopo 6 settimane in ospedale, stasera siamo riusciti a portare Luca a casa!

Inizialmente pensavamo che avrebbe dovuto fare la riabilitazione in ospedale, ma Luca aveva altre idee ed era determinato a tornare a casa! Per fortuna, siamo in grado di somministrargli i farmaci, mantenere il suo tubo di alimentazione (con la bocca ancora cablata) e averlo a casa. Il piano prevede di portarlo in ospedale tutti i giorni per la riabilitazione e la terapia.

Siamo davvero grati che la lesione cerebrale traumatica di Luca finora non abbia compromesso la sua capacità di funzionare. Siamo fiduciosi che il futuro continuerà a mostrare progressi. Con la riabilitazione presso l'ospedale originale della Utah Valley, i medici hanno anche la possibilità di monitorare le numerose fratture e lesioni dei tessuti molli di Luca. Potrà togliersi il gesso dal polso, così come i perni. La mandibola cablata verrà rimossa entro le prossime due settimane speriamo prima! Potrebbe persino riuscire a togliere uno dei supporti per le gambe che ha indossato. Con il tempo vedremo come Luca riuscirà a compensare con il suo braccio e la mano destra. Dovrà impegnarsi e creare nuove vie neurali per muovere il braccio. Soffre costantemente di dolore nervoso, ma è pronto a lavorare. È entusiasta di alzarsi dal letto, iniziare una routine di terapia (ergoterapia, fisioterapia, logopedia...) e vedere dei progressi.

Luca ha ora un nuovo telefono con lo stesso numero, quindi chiamatelo o inviategli un messaggio. Apprezza molto i pensieri e l'affetto. So che ha detto che con tutte le sue cicatrici sembra una mappa del mondo... forse sarà il suo costume per Halloween 😅 Vi vogliamo bene. Grazie mille.

Oct 20 - I AM HERE WITH LUCA! Woot woot!

So we did the third surgery here at U of U, and are planning to return down to Provo next week after recovering a little. This last operation was not as intense or long as the last ones. Luca is feeling "ready to get out of the hospital". 

We have a good set-up for medication and pain management so that Luca can keep healing. The shoulder nerves were reattached during this procedure to help with future shoulder movements. Although there was damage present, the surgeon is optimistic with Luca's health/youth that he will heal and compensate for the difficulties that arise.

It has been amazing spending time with Luca (in-person), especially with his pain managed. We get to talk, listen to music and books, and just spend time together. He is so funny and sweet.

Luca says "I really love you all. I am so grateful for all the messages and support I have received. It is what is keeping me alive. Hopefully I will be out of this hospital soon and can thank each of you in person with a big hug!"

🍕 20 ottobre - SONO QUI CON LUCA!

Abbiamo fatto il terzo intervento qui all'Università dello Utah e stiamo pianificando di tornare a Provo la prossima settimana dopo un breve periodo di recupero. Quest'ultimo intervento non è stato così intenso o lungo come i precedenti. Luca si sente 'pronto a uscire dall'ospedale'. Abbiamo una buona gestione dei farmaci e del dolore in modo che Luca possa continuare a guarire. Durante questa procedura, i nervi della spalla sono stati riattaccati per aiutare nei futuri movimenti della spalla. Anche se c'era danno presente, il chirurgo è ottimista sulla salute/giovinezza di Luca e confida nella sua capacità di guarire e compensare le difficoltà che possono insorgere.

È stato incredibile passare del tempo con Luca (di persona), specialmente con il suo dolore sotto controllo. Possiamo parlare, ascoltare musica e libri e semplicemente stare insieme. È così divertente e dolce.

Luca dice: "Vi amo davvero tutti. Sono così grato per tutti i messaggi e il supporto ricevuti. È ciò che mi tiene in vita. Spero di uscire presto da questo ospedale e potervi ringraziare di persona con un grande abbraccio!"

Oct 16 - Sorry...long delay. 

Here is what has been going on with Luca:

• Last Thursday (12th) the neurosurgeons at U of U did a preliminary operation to see the connectivity of Luca's right brachial plexus (nerves of shoulder to arm and hand). The hope was that one of the nerves would be conductive and long enough to attach to his spine. 

Fingers are one of the most nerve-sensitive areas in our bodies. This area can sense small differences in temperature, pressure, and pain compared to others areas on our bodies, like our side or back. This was why it was important to try and find a nerve from this area of the body to connect back to the spinal cord; it improves the chance of dexterity and sensitivity. However, the doctors sadly did not find any nerves in the brachial plexus that would fit the criteria needed. There was too much damage.

•  Last Friday (13th) the second surgery for Luca took place to connect nerves from the right side of his torso--under his armpit--to his arm. This procedure is meant to help Luca be able to move his arm. It took over 10 hours for the neurosurgeons to finish the task. Since the nerves are merely glued together, Luca can't move that right arm in anyway to compromise the new connections. Things are progressing, but there is a lot of pain in the journey. Luca feels constant burning through his super sensitive right hand and had a really rough night after the operation due to feeding tube problems and lack of pain medicine. It was also really difficult for us, the fam, to see him in that state.

• Luca was able to remove the neck brace since his neck ligaments have recovered a bit. It is also helpful since the operations are going up along his neck and they now won't have to worry about the brace.

• For now, Luca is still recovering from the latest procedure at the university hospital. The next plan is another surgery already. It is scheduled for Friday (20th). This surgery is meant to connect nerves in his shoulder so that he may be able to move it again! We are hopeful it will go well.

🍕 16 ottobre - Scusate...lungo ritardo.

Ecco cosa è successo a Luca:

• Giovedì scorso (12 ottobre) i neurochirurghi dell'Università dello Utah hanno eseguito un intervento preliminare per valutare la connettività del plesso brachiale destro di Luca (nervi dalla spalla al braccio e alla mano). La speranza era che uno dei nervi fosse conduttivo e abbastanza lungo da poter essere attaccato alla sua colonna vertebrale. Le dita sono una delle aree più sensibili ai nervi nel nostro corpo. Questa zona può percepire piccole differenze di temperatura, pressione e dolore rispetto ad altre parti del nostro corpo, come il fianco o la schiena. Per questo era importante cercare di trovare un nervo da questa zona del corpo da ricollegare al midollo spinale; migliora la possibilità di destrezza e sensibilità. Tuttavia, i medici purtroppo non hanno trovato nessun nervo nel plesso brachiale che soddisfacesse i criteri necessari. C'era troppo danno. 

• Venerdì scorso (13 ottobre) si è svolta la seconda operazione per Luca per connettere i nervi dal lato destro del suo torace - sotto l'ascella - al suo braccio. Questa procedura è pensata per aiutare Luca a muovere il braccio. I neurochirurghi ci hanno messo oltre 10 ore per completare il compito. Dato che i nervi sono solo incollati insieme, Luca non può muovere in alcun modo quel braccio destro per compromettere le nuove connessioni. Le cose stanno progredendo, ma c'è molto dolore nel percorso. Luca sente un bruciore costante nella sua mano destra estremamente sensibile ed ha avuto una notte davvero difficile dopo l'operazione a causa di problemi al sondino gastrico e mancanza di antidolorifici. È stato davvero difficile anche per noi, la famiglia, vederlo in quello stato. Per ora, Luca si sta ancora riprendendo dall'ultimo intervento all'ospedale universitario. 

• Luca è stato in grado di togliersi il collare cervicale poiché i suoi legamenti del collo si sono ripresi un po'. È anche utile poiché gli interventi stanno procedendo lungo il suo collo e ora non dovranno preoccuparsi del collare. 

• Il prossimo piano prevede un'altra operazione già in programma per venerdì (20 ottobre). Questa chirurgia è pensata per connettere i nervi nella sua spalla in modo che possa muoverla nuovamente! Siamo fiduciosi che andrà bene.

Oct. 9 - Today we moved to University of Utah hospital! Luca is feeling good, especially with things progressing! Mirai made him a nice blanket; her and Giorgia helped Luca get settled in his new room. The doctors came in and said they are planning to have an investigatory surgery (to determine the damage...etc.) on Thursday. Then on Friday they will take functioning nerves and attach them to his right brachial plexus (arm bundle of nerves). 

Other good news! After checking with more x-rays, they are hoping to take off the neck brace, and remove the pins from his left finger this week. We are so glad that things are finally moving along after not knowing much last week.

I'll keep you updated about the nerve surgery and when he moves back down to Provo for Rehab, Therapy, and any visits? *Hint hint*

🍕 9 ottobre - Oggi ci siamo trasferiti all'ospedale dell'Università dello Utah! Luca si sente bene, specialmente con i progressi che stanno facendo! Mirai gli ha fatto una bella coperta; lei e Giorgia hanno aiutato Luca a sistemarsi nella sua nuova stanza. L'operazione al nervo dovrebbe seguire a breve!

Altre buone notizie! Dopo ulteriori controlli con i raggi X, sperano di togliere il collare cervicale e rimuovere i perni dal suo dito sinistro questa settimana. Siamo così felici che finalmente le cose si stiano muovendo dopo non aver saputo molto la scorsa settimana.

Vi terrò aggiornati sull'intervento al nervo e quando tornerà a Provo per la riabilitazione, la terapia e eventuali visite?😉

Oct. 8 - This weekend has been a waiting game. Luca has gotten more aware of his situation, which helps us to interact with him; however, Luca also realizes how much damage his body has endured, and really struggles not being able to do normal, independent functions. Understandably, he is really down.

University of Utah hospital estimates a bed availability for Luca and his nerve surgery Tuesday! We are hopeful for this next operation, and pray it will make a difference in Luca using his right arm and hand. I will keep you updated with any changes.

If you are still reading this, wow! Thank you! At times I feel these updates are for my own sanity...either way, I want Luca to know he is loved, in our thoughts, and certainly in our prayers. 🍕 8 ottobre - Questo fine settimana è stato un gioco di attesa. Luca sta diventando più consapevole della sua situazione, il che ci aiuta a interagire con lui; tuttavia, Luca si rende anche conto del tanto danno che il suo corpo ha subito e fatica molto a non poter svolgere funzioni normali e indipendenti. Comprensibilmente, è davvero giù.

L'ospedale dell'Università dello Utah prevede la disponibilità di un letto per Luca e la sua chirurgia al nervo martedì! Nutriamo speranze per questa prossima operazione e preghiamo affinché faccia la differenza affinché Luca possa usare il suo braccio e la sua mano destra. Vi terrò aggiornati su eventuali cambiamenti.

Oct. 5 - What a difference when Luca's pain is managed! He is Luca again! Super kind and with greater clarity of mind.

The next step we are hoping for is to have a nerve surgery to reattach functioning neurons to his spine again. The hope is that Luca may have better function down his arm and, specifically, into his hand. (See Oct. 2 for his specific diagnosis and an informative video). We are waiting for authorization to have Luca moved to the UofU hospital; this is the location of the only neurosurgeon qualified in Utah to perform this specific operation for our boy. It seems we are mainly waiting on bureaucratic paperwork stuff...yeah not real technical heh. We will just see how long it takes!

🍕 5 ottobre - Che differenza quando il dolore di Luca è sotto controllo! È di nuovo Luca! Super gentile e con maggiore chiarezza mentale.

Il prossimo passo che speriamo si realizzi è un intervento chirurgico ai nervi per riattaccare neuroni funzionanti alla sua colonna vertebrale. Speriamo che Luca possa avere una migliore funzionalità lungo il braccio e, in particolare, nella mano (vedi 2 ottobre per ulteriori informazioni e un video informativo). Stiamo aspettando l'autorizzazione per far spostare Luca all'ospedale dell'Università dello Utah; questo è il luogo in cui si trova l'unico neurochirurgo qualificato nello Utah per eseguire questa specifica operazione per nostro figlio. Sembra che stiamo aspettando principalmente a causa di questioni burocratiche... sì, non proprio tecniche eh. Vedremo quanto tempo ci vorrà.

Oct. 4 - It was a really rough night last night. Luca, after his surgery, was pretty agitated and ended up pulling his feeding tube (the one in his nose) right out. This not only feeds Luca, but it is also how he receives his sleep, anxiety, and pain medication! I don't know all the details beyond the fact that it was a hell of a night, not just for Luca in excruciating pain, but also for my dad trying to keep him calm and find out what could be done. 

Beyond the excruciating night, x-rays were taken to ensure his night of altercations didn't upset the newly operated shoulder. Luckily, it is in good shape.

Once Luca finally had some pain meds he was able to sleep.

🍕 4 ottobre - È stata davvero una notte difficile quella di ieri. Luca, dopo l'intervento, era piuttosto agitato e ha finito per tirare fuori il suo sondino nasogastrico. Questo non solo nutre Luca, ma è anche il modo in cui riceve sonno, farmaci contro l'ansia e antidolorifici! Non conosco tutti i dettagli oltre al fatto che è stata una notte terribile, non solo per Luca in un dolore lancinante, ma anche per mio padre che cercava di mantenerlo calmo e capire cosa potesse essere fatto.

Oltre alla notte dolorosa, sono stati fatti raggi X per assicurarsi che tutti i suoi movimenti della scorsa notte non abbiano disturbato la spalla appena operata. Per fortuna, è in buone condizioni.

Una volta che Luca ha finalmente preso alcuni antidolorifici, è riuscito a dormire.

Oct. 3 - From yesterday's MRI, it was also determined that Luca has a grade 3 AC joint separation/sprain. Basically this means that the ligament connecting Luca's collar bone and the top of his shoulder blade has been torn, and a couple other ligament connections as well.

However, the good news is that they operated to reconnect the breaks today. Luca is hopeful that this will help with some shoulder movement.

🍕 3 ottobre - Dall'MRI di ieri, è stato anche determinato che Luca ha una separazione/sprain dell'articolazione acromion-clave di grado 3. Fondamentalmente questo significa che il legamento che collega la clavicola di Luca alla parte superiore della sua scapola è stato strappato, insieme ad alcuni altri legamenti.

Tuttavia, la buona notizia è che oggi hanno operato per riconnettere le fratture. Luca è ottimista che questo possa aiutare con alcuni movimenti della spalla.

Oct. 2 - It has been a rollercoaster of emotions. Today was spent trying to understand Luca's nerve damage and unmoving right arm. With his mouth wired, it is difficult for the MRI to capture clear images, so Luca had to stay very still in the small space. It took a couple tries.

Sadly, the results were not what we had hoped.

Turns out the C7 and C8 nerves (from the spine) have been ripped off (avulsion) which is considered the most severe type of nerve injury. There were areas of irregularity for nerves C4, C5, and C6, but it was hard to see. It could be a stretch injury or another type of disruption (ruptured nerve, not attached...)

I've learned a lot about the functions of specific nerves from this video (maybe speed it up though):

We do have reason to hope! There is the possibility of surgery to reattach healthy, functioning nerves in the future to compensate for the damage. Although they predict it will never be the same, we hope that he can learn to use his arm and hand again.

🍕 2 ottobre - È stato un'altalena di emozioni. Oggi è stata dedicata a cercare di capire il danno ai nervi di Luca e il suo braccio destro immobile. Con la bocca bloccata dai fili, è difficile per la risonanza magnetica catturare immagini chiare, quindi Luca ha dovuto restare molto fermo nello spazio ristretto. Ci sono voluti alcuni tentativi.

Purtroppo, i risultati non sono stati quelli che speravamo. Sembra che i nervi C7 e C8 (dalla colonna vertebrale) siano stati strappati via (avulsione), il che è considerato il tipo più grave di lesione nervosa. C'erano zone di irregolarità per i nervi C4, C5 e C6, ma era difficile vedere. Potrebbe essere una lesione da stiramento o un altro tipo di interruzione (nervo rotto, non attaccato...).

Abbiamo motivo di sperare! C'è la possibilità di un intervento chirurgico per riattaccare nervi sani e funzionanti in futuro per compensare il danno. Anche se prevedono che non sarà mai uguale, speriamo che possa imparare a usare di nuovo il suo braccio e la sua mano.

Nov 23 - Happy Thanksgiving to you lovely humans that celebrate the turkey holiday. Happy Thursday to others!

We have many reasons to be thankful. The recovery is slow, as the surgeons warned us, but Luca is so strong and helps us see the good in each day. The nerve regrowth and reconnection has been very painful, especially as we continue to decrease medication and look for alternatives for managing the pain.

More surgeries are upcoming in the progressing months. Although Luca does not have his mouth completely wired shut anymore, there is still a lot of hardware (wires and metal) that can only be removed surgically. That's the first to come, maybe even in December. He also has a tear on his PCL and meniscus that will need to be addressed.

The family had a great Thanksgiving with delicious food, lots of company, and karaoke (as seen in the picture above). I personally am super excited for Christmas since Noah and I will be driving back to Utah to see the fam❤️

🍕 23 novembre - Buon Ringraziamento a voi adorabili esseri umani che festeggiate la festa del tacchino. Buon giovedì agli altri!

Abbiamo molte ragioni per esserne grati. La ripresa è lenta, come ci avevano avvertito i chirurghi, ma Luca è così forte e ci aiuta a vedere il bene in ogni giorno. La crescita e la riconnessione dei nervi sono state molto dolorose, specialmente mentre continuiamo a diminuire la medicazione e cerchiamo alternative per gestire il dolore.

Ci sono altre operazioni in arrivo nei prossimi mesi. Anche se Luca non ha più la bocca completamente slegata, c'è ancora molto hardware (filo e metallo) che può essere rimosso solo chirurgicamente. Questo sarà il primo intervento, forse già a dicembre. Ha anche una lacrima al legamento crociato posteriore (PCL) e al menisco che dovranno essere affrontate.

Grazie ancora per l'amore e per le vostre verifiche! La famiglia ha trascorso un ottimo Ringraziamento con cibo delizioso, molta compagnia e karaoke (come si vede nella foto qui sopra). Personalmente, sono molto entusiasta per Natale, poiché io e Noah torneremo a guidare fino in Utah per vedere la famiglia ❤️
