As a Link Worker, your role is to help clients successfully attend their first program, stay engaged over time, and document meaningful outcomes.
This chapter will guide you through:
Preparing for a client's first program visit
Conducting 3-month, 6-month, and 12-month follow-ups
Updating client records and documenting outcomes
After completing the client consent form, client information form, and co-creating a participation plan, the next step is to help the client attend their first program.
This is often the clientβs first real experience with your centre.
A positive first visit can strongly influence long-term participation.
Step 1
Review the clientβs participation plan.
Ask yourself:
What programs were selected?
Are there mobility concerns?
Are there language or accessibility needs?
Step 2
Confirm the first visit.
Contact the client and confirm:

transportation needs

Step 3
Reduce first-visit anxiety.
Offer:
a welcome introduction

volunteer support

At approximately 3 months, conduct a supportive, informal check-in.
The goal is to identify barriers early and encourage continued participation.

Contact the client.
Example questions:

βIs anything making participation harder?β

Document barriers.
