- Completed application (standard written application or color printed online application)
- Payment to OSBI (cashiers check, money order, online payment)
- Payment to Sheriff Department $25 (cashiers check or money order)
- 2 passport sized photos (not needed if photos were uploaded online and printed in color)
- Valid identification (drivers license, state issued ID, passport)
- Original certificate from an OSBI approved instructor dated within the last 3 years
