Integrative Medical Consultation (Licensed States)  Iowa and Arizona  $250 
Follow-Up Visit Fees 75-150 dollars based on  visit type
Integrative Hormone Optimization Program not including the BioTE Pellets  (6 months)

$250 dollars for first visit followed by $175 / month

Program includes initial comprehensive visit with a follow up lab review visit in the first  month  with integrative treatment planning . This is followed by  monthly follow up visits for  monitoring progress and any required changes in diet lifestyle  and medication or supplements  dose adjustments.  The plan includes treatment planning  and  Integrative diet and  lifestyle guidance, one follow up  visit every month and messaging support

The plan does not  include lab or supplements cost. Labs can be ordered through insurance or direct pay through clinic,  based on  insurance coverage. Supplements ordered separately.

BIOTE® Pellet Therapy Pricing

Initial consult is $250 for comprehensive assessment for eligibility and risk assessment for pellet insertion

Women’s Pellet Insertion   $375 per procedure (Every 3–4 months)

Men’s Pellet Insertion $725 per procedure (Every 5–6 months)

Lab and supplement charges are separate , not included in the pellets charges

Initial Hormone Lab Panel — Men $180 (cash pay) or billed to insurance if preferred  based on insurance coverage
