Sworn Statement of Capital;
Proof of Registration i.e. Certificate of Registration
Issued by the Security Exchange Commission (SEC) for all kinds of corporations (Certificate of Registration and Articles of Incorporation)
Issued by the Cooperative Development Authority (CDA) for cooperatives;
Issued by Department of Trade and Industry (DTI) for sole proprietor;
Proof of right of applicant to use location as business address, which may include any of the following:
If owned, proof of ownership such as Transfer Certificate of Title or Tax Declaration;
If not owned by applicant, the following documents are required:
Notarized Lease Contract, or Memorandum of Agreement
Lessor's Business Permit
Or written consent of property owner in a public document with accompanying valid IDs and three (3) specimen signatures.
Locational Clearance issued by the City Planning and Development Office;
Picture of front of establishment and screenshot of location with coordinates using Google Maps
Fire Safety Inspection Certificate for Occupancy, valid in the last nine (9) months, with Affidavit of Undertaking that there had been no substantial changes made on the building/establishment given the FSIC as required by the Bureau of Fire Protection (BFP);
Occupancy Permit issued by the City Engineer's Office;
Certified List of Employees or Positions to be filled up;
Valid Special Power of Attorney or Secretary's Certificate designating the authorized representative of the registered owner if the application cannot be made in person;
And all other requirements provided for by City Ordinances and National Laws (including but not limited to)
Contractor's License from Philippine Contractors Accreditation Board
FOR RENEWAL APPLICATION
Proof of annual gross sales/receipts of immediately preceding year which may include:
Audited financial statements or unaudited financial statements for those who are not required to file AFS with the BIR or;
Sworn Statement of Gross Sales or Receipts
Income Tax Returns;
Certified List of Employees;
Proof of right of applicant to use location as business address, which may include any of the following:
If NOT OWNED BY APPLICANT, the following documents are required:
If RENTING:
UPDATED/VALID Notarized Lease Contract, or Memorandum of Agreement;
