Lesson Suspension / Discontinuation Form

If, after completing the Suspension Form, you want to make sure your lessons are reinstated for the following billing period, make sure to complete the Lesson Resumption Form, as well.

PLEASE also NOTE: Your discontinuation will only go into effect for the end of this current month:

  • If you have submitted the online form, AND
  • If you have received acknowledgement of notification, AND
  • Both have occurred prior to next month’s billing date (usually the 20th of the month; or the first business day, thereafter should the 20th fall on a weekend or holiday).
