Injury Reporting: 

When an incident occurs at a race, practice or other team or League event that results in the following:

○  Referral or visit a medical facility

○  Unable to complete or attend team practices, train or race

○  Unable to attend school or work

○  Unconsciousness or potential brain injury

○  More than minor First Aid is performed

This form must be completed by the Coach for a practice or team event and by the Parent/Guardian for a race event: Colorado League Injury Report Form

How do I file a claim?

Accidental medical: The registered injured party and/or legal guardian must submit an incident report form. 

1. This form must be submitted. Written notice of incidents must be submitted within 30 days of the incident.

Colorado League Participant Injury form

A confirmation email with instructions on the claim filing process will be sent once the form is submitted.

This form is also available on the Colorado League Rules and Insurance website page. Injured parties are then contacted by the Insurance Company and the League.
