Forms

Per VHSL rules, all athletes must have a VHSL Athlete Participation physical and concussion form completed after MAY 1 of the current year. The physical is valid from the date of the physical to June 30 of the following school year. Please turn in all forms to L.C. Bird High School Athletics.
