Dues & Forms

EVERY SEASON:  Fill out USAU Medical Forms and upload them here, or send them to Coach Jana

• Enter "Lafayette High School ultimate club coaches" as the Name of Chaperone

• We do NOT use the CHSAA forms that other sports use

EVERY CALENDAR YEAR:  Get a USA Ultimate Membership and email Coach Jeff your membership number if you are new to our program

There are several membership tiers available - feel free to choose the one that is best for your situation (note: these change over time)

• Youth

  • Often the most appropriate for players on our team

  • Should be $40

  • Valid for one calendar year

• Youth Intro

  • Same as Youth, but only valid for the first time ever getting a membership

  • Should be $20

• Youth Fall + Next Year Too

  • This is sometimes a great deal - available starting in mid-September

  • Should be $60

  • Valid for the Fall plus the entire next calendar year

EVERY SEASON:  Pay your Team Dues on CheddarUp

This is $150 per season and will cover field rentals, tournament fees, team league registration, and related expenses

Your coaches are volunteers and do not get paid (unless we have some money left over)

Financial hardship should never prevent anyone from participating on our team.  Scholarships are available for students who need support.  Reach out to Coach Jeff for more information
