Athletics Forms

Before you can participate in Valley sports, each athlete will need to complete the forms listed below. Your sports physical must be submitted into "DragonFlyMax".

Create and complete ALL REQUIRED forms in "DragonFly" (https://www.dragonflymax.com)

• You will need to complete the "Concussion for Students" 

  •  1) Log onto https://nfhslearn.com/ and search for "Concussion for Students", register and complete.

  •  2) Once complete, in DragonFly, link your NFHSLearn account to your DragonFly account and the certificate will automatically transfer over.

• Upload your annual sports physical, must be dated after April 1, 2025.

Completed NMAA Physical Packets must be submitted into "DragonFlyMax". Sports physical packets can be downloaded and printed. Athletes who choose to play in school sports for 2025-26, must have a sports physical dated: April 1, 2025 or sooner. No sports physicals will be accepted if they are older than March 31, 2025.
