Interest Form

All transportation to and from away contests/events is the responsibility of and will be provided by the school district. All student-athletes are required to travel to and from away contests in transportation provided by the school district. Student-athletes should not be transporting themselves to or from away contests. If a special situation occurs, the student-athlete must request approval from the athletic director 24 hours prior to the contest. Forms are available in the Athletic Office and on the athletic web site and must be signed by the student-athlete, his or her parents or guardians, and the athletic director prior to being given to their coach. In these cases, the student-athlete will only be released to their own parent/guardian.

Alternate Transportation Form Request

Any student-athlete wishing to participate on one of our athletic teams in grades 7-12 is required to complete required paperwork to try out by August 1. Other than the physical form, all of the forms can be typed on, printed out and then signed before turning them into the CHS Athletic Office. The below forms are required to be completed and submitted to the Athletic Department before tryouts.
