2. That at the time of our child’s birth, we were not yet married to each other, and as a result, the child was registered as illegitimate in his/her birth certificate;

4. That by virtue of our valid subsequent marriage and pursuant to Article 177 of the Family Code of the Philippines, our child [Child’s Full Name] has been legitimated by operation of law and is now deemed our legitimate child, with all rights and privileges as such;

That we are executing this joint affidavit to affirm the legitimation of our child and to request the necessary annotation or correction of records to reflect his/her new civil status in the appropriate government offices, including but not limited to the Philippine Statistics Authority (PSA) and the Local Civil Registry Office; and

6. That we further declare that the foregoing statements are true and correct to the best of our knowledge and belief.
