I am no longer taking insurance except for Medicare, and Cen-Cal.
Please call me directly regarding appointment availability.
For all other insurance companies, I will provide you a "Superbill" that you can attach to your own claim forms for reimbursement directly from your insurance company. My fee must be paid directly to me in full. Submission of a claim form is not a guarantee of payment.
Payment
My fee is $150.00 a session. I take cash or checks. I do not take credit cards for payment.
Cancellation Policy
If you cancel with less than 24 hours notice you will be billed $50. If you forget or don't show up for your appointment you will be billed $50. Your health plan cannot be billed for this fee.
