Navigate the link given above. Then, access your class program via the dashboard:

*For mobile users: the dashboard is located under the home icon.

*For PC/tablet users: the dashboard is located beside the home icon.

Click on the link under the names of your respective instructor to be prompted to their evaluation form.
You will be required to input your student identification number in each form (for identification during the signing of clearance. Rest assured that this information will be strictly confidential)
See to it that all your instructors will be evaluated.
