A free safety program that helps first responders quickly access critical medical and emergency contact information during a vehicle emergency.
Participants place a Yellow Dot decal on their vehicle and keep a completed information form in the glove box.

Q: Where do I get Yellow Dot Program materials?
A: Request a Yellow Dot packet (sticker and brochure/personal information form here)
Q: What materials are needed to participate in the Yellow Dot Program?
A: For each vehicle you will need a Yellow Dot decal/sticker and Personal Information forms for each driver and passenger.
Q: Can I get Yellow Dot Program materials for more than one vehicle?
A: Yes, you can get a Yellow Dot decal and Personal Information forms for every vehicle you drive. If you need materials for more than 3 vehicles - please email katc@louisville.edu
Q: Can I fill out the Personal Information form for passengers in my vehicle?
A: Yes, you should complete the personal information form for any other driver(s) or passenger of your vehicle, especially if they have a specific medical need (i.e. autistic, difficulty communicating in stressful situations, medical allergy, diabetes, heart problems).
Q: Where does the Yellow Dot decal go on my vehicle?
A: The Yellow Dot decal should be placed on the driver’s side rear window of the vehicle and the brochure/medical information sheet in the glove box.
For motorcycles - Place the decal on the "triple tree" of the motorcycle and put the packet in the saddle bag/compartment.
Q: What happens if I don't have my photo on the medical form?
A: The photo is crucial to the success of the program. Yellow Dot is about saving those precious seconds immediately following a serious incident. Having a photo identifying to whom the medical form belongs will save time and possibly your life. Please make sure a photo is included with each Personal Information form.
Q: What happens to my personal information?
A: Use of this form is voluntary. By using this form, you understand that first responders and medical personnel will use this information as they see fit.
Q: Is my social security number needed on the medical information form?
A: No, do not include social security number, credit card information or any other sensitive information.
Q: Do I have to pay to participate in the Yellow Dot program?
A: No, the program is FREE!
Q: Can I request a large number of Yellow Dot materials for a community event?
A: ABSOLUTELY! We ask that you email katc@louisville.edu at least 2 weeks in advance as these materials are made in house.
We estimate that the Yellow Dot materials cost approximately $2 each to create and mail so please be mindful of the number you are requesting.
TIP: We recommend having an event or segment of an event where participants actually fill out the brochure/medical form and then everyone goes to their vehicles and adheres the decal on the rear windshield.
